Employer and salary information

Throughout your training programme you will be employed a Lead Employer Organisation (LEO), which in our area is the Royal Free London NHS Foundation Trust.

The LEO provides a contract of employment to cover the whole period of General Practice Training. They deal with all the employment issues for GP Trainees throughout their training programmes, whether they are based in hospital or a GP practice.

You will be paid according to the ST pay scale – a trainee joining the VTS after completion of Foundation training will join the pay scale at the minimum point.  In GP settings, your basic salary will be topped up by the so-called “GP Registrar supplement”. Banding in hospital posts as well as the GP Registrar supplement are subject to regular review by the Doctors and Dentist Review Body (DDRB).

Any queries relating to salary or terms and conditions of service whilst undertaking training should be taken up with the LEO.

Long term sickness/leave

If you are on long term sickness/leave you must also inform the LEO and your TPD. This may have an impact on your pay and CCT date.

Safeworking and exception reporting

You can submit exception reports (ERs) of additional hours worked which may be recompensed with time off in lieu or payment. If there is a repetitive pattern within a post, a work schedule review will be requested to identify rota gaps/enact changes. Trainees can also submit ERs for missed scheduled educational activities (e.g. missing your VTS teaching).

Disability and Reasonable adjustments

If you have a disability that might require adjustments to be made to support your working you must make the TPDs aware who can refer you to the occupational health (OH) service for GP trainees.

Less than full time training (LTFT)

This applies if you wish to work part-time hours. This will mean the programme will take proportionately longer to complete, which will affect your placement and CCT dates. The application is managed in the PGMDE Support Portal.

Target Enhances Recruitment Scheme (TERS)

This is an initiative that will offer a one-off payment to GP Specialty Trainees committed to working in a select number of training locations that either have a past history of under-recruitment or are in under-doctored or deprived areas. You apply for this during the GP training recruitment process.

National Medical Performers List (NPL) for GP Placements

All GP registrars need to be on the National Medical Performers List (NPL) to be able to see patients in General Practice. HEE will supply NHS England with details of all GP registrars who need to be added to the list each August and February. NHS England will confirm their inclusion to Primary Care Services England (PCSE) who will then notify each GP registrar by email (by the end of October for August starters, and the end of April for February starters). Anyone who has not been added to the list by 12 weeks after their first day based in GP (‘the grace period’) has to stop seeing patients.

Indemnity

The government’s state-backed clinical negligence scheme for general practice (CNSGP) is run by NHS Resolution (funded by NHSE) and covers clinical negligence arising from NHS patient care. Medical Indemnity Insurance is provided by GPSTs by Medical Defence Union Limited (MDU) and the associated costs of this cover is paid for by HEE. This provides cover for medico-legal advisory support and clinical negligence for GPST’s working in organisations outside the NHS. This does not preclude individual from taking out additional insurance or professional indemnity cover if they wish from a defence organisation, trade union or other body of their choosing. For any additional work undertaken outside your training programme you must ensure that you purchase additional cover. If you decide to opt out of the MDU indemnity agreements you will need to inform HEE.
